The issue is resolved in QuickBooks Pro 2012 R7.

Relevé 1 slip

Not a Relevé 1 form. Include the accent, even in English. Don’t shorten to Relevé or RL-1. If there are severe space constraints, it’s OK to shorten to Relevé 1, but this should be avoided.
